Partnerships play a significant role in the work that we do and the success of the organization. Each day, we are excited and humbled to have the opportunity to help those who rely on our programs and services, and we recognize our work can only happen because of the support we receive. One such partnership that is making a meaningful difference is our new collaboration with the Boulder Community Health Foundation.
Boulder Community Health Foundation: Community Collaboration Grant
The Community Collaboration Grant funds care management for individuals referred to JFS by the Boulder Community Health Foundation, focusing on older adults facing significant social and economic challenges and those with complex needs.
JFS Boulder’s Care Management program aims to address critical issues affecting older adults in Boulder, including social isolation, the desire for aging in place, chronic health conditions, and access to community resources. The program is designed to provide comprehensive support and services tailored to the needs of this population.
With the Community Collaboration Grant, we can expand our current care management services and provide emergency assistance, social engagement through the Friendly Visitors program, healthcare coordination, transportation, meal deliveries, and holiday meal distributions. In addition, we will facilitate regular family meetings and provide access to Caregiver Support Groups.
This grant will significantly enhance the quality of life for older adults in Boulder County, ensuring they receive the care and support they need.
